Sponsorship Request form (for donations) In order to receive donations for a team or club under the 501c(3) non-profit status of the Clairemont High School Foundation, CHS Clubs (academic, artistic or athletic) must submit this form prior to the monthly Foundation Board meeting the 3rd Tuesday of the month. If you would like a webpage with a pay pal button to receive donations, you can request that with this form. Grant Request Form Use the Grant Request form to request the CHS Foundation to support your project. CHS Foundation works to fund grants that provide the extra resources your group requires for a much-needed program or purchase necessary supplies. We seek to approve grants that will have a positive impact on the entire CHS community, both directly and indirectly. Grants are typically funded up to $250. CHSF Deposit Form Any money raised under the CHS Foundation umbrella must be submitted to the treasurer, to be deposited into the Foundation account, with this form. Please have two signatures on the form verifying an accurate money count. Payment Request Form Use this form to request a withdrawal of funds from your club, team or academy line for approved purchases, or for reimbursement of approved purchases following a grant request submitted with receipts or invoices.
