Grant/Sponsorship request form
In order to fundraise under the 501c(3) non profit status of the the Clairemont High School Foundation, CHS Clubs (academic, artistic or athletic) must submit this form, in person, to the Foundation Board at a monthly meeting. It outlines the details of the event and is signed by the Principal, Advisor/Coach and a Student Representative. The request will be voted on the evening of the submittal.
Booster Fundraising Request form
This form is required for any and all Booster, PTSA, Foundation, etc. that would like to have a fundraising event.
Cash Verification Form
Any money raised under the CHS Foundation umbrella must be submitted to the treasurer, to be deposited into their account, with this form. Please have two signatures on the form verifying an accurate money count.
Payment Authorization Form
Use this form to request a withdrawl from your club,team or academy account for approved purchases
